Management system procedures and forms

Introduction and purpose

procedures and forms

The purpose of the incident investigation and reporting procedure is to outline your organization’s methodology to establishing the processes required for an effective incident and near miss response programme which methodically examines all undesired events that have or could have resulted in physical harm to interested parties or result in damage to property.

The intent of any resulting investigation is to establish the facts and circumstances related to the event in order to determine the root-cause and develop remedial action to control the risk.

Application and scope

The incident investigation and reporting procedure has been developed to assist in meeting the requirements of Clause 10.2 of ISO 45001:2018 - Incident, nonconformity and corrective action.

The scope of the incident investigation and reporting procedure applies to all incidents that are associated with current health and safety requirements. This procedure is applied when taking action to investigate and mitigate the causes and consequences arising from incidents and includes the initiation and completion of corrective actions and confirmation of the effectiveness of actions taken.
