The purpose of the change management procedure is to establish a standard approach to applying changes to production and the QMS. Changes require thorough planning, careful monitoring, and follow‐up evaluation to reduce negative impact to the user community and to increase the value of vital information resources.

This is done through a formal process of recording, assessment, authorization, scheduling and comprehensive communication around all changes.

Application and scope

The change management procedure has been developed to assist in meeting the requirements of Clause 6.3 of ISO 9001:2015 - Planning for Changes and applies to all physical, process, procedural, personnel and organizational changes within existing operational assets.

The change management procedure provides a structured approach to managing changes within an organization, ensuring that changes are made in a controlled and efficient manner while minimizing disruptions and risks.

Change request form

The change request form is a document used to formally propose a change within your organization. It helps ensure that all necessary information is gathered and reviewed before any change is approved and implemented. A properly completed change request form provides clarity on what the change involves, why it is necessary, and what impact it may have.

This form ensures that the change request process is standardized, helping to minimize risks and maintain control over any changes that are made within your organization.

Change request log

The change request log is a document or database used to track all change requests within an organization. It serves as a central record to monitor the status of each change, ensuring that changes are handled in a controlled and organized manner. The log contains details about the change, its impact, approval status, and implementation progress.

A well-maintained change request log helps ensure the successful and organized implementation of changes, minimizing risks and disruption to business operations.
