Hazard identification and assessment procedure

ISO management system procedures and forms

Introduction and purpose

procedures and forms

The purpose of the hazard identification and risk assessment procedure and the hazard identification register is to provide your organization with an ongoing and proactive methodology for identifying and evaluating operational hazards.

Hazard identification and assessment is the process by which health and safety risks are identified, captured, and evaluated to determine the most suitable control measure. The hazard identification and assessment process is used for routine and non-routine activities and for assessing new processes, changes in operation, products, or services as applicable.

Application and scope

The hazard identification and risk assessment procedure and forms have been developed to assist in meeting the requirements of Clause 6.1.2 of ISO 45001:2018 - Hazard identification and assessment of risks and opportunities.

Hazards exist at all organizational levels and are detectable through many sources, including reporting systems, inspections, audits, brainstorming sessions, and expert judgment. The hazard identification process aims to proactively identify hazards and define their key characteristics before they lead to accidents, incidents, or other safety-related occurrences.

The hazard identification and risk assessment procedure includes four essential form templates referenced from the procedure, supplied in .docx and .xlsx formats that can be retained as hard or soft copies.

Hazard identification register

Set out in a table format; the hazard identification register can present a great deal of information in just a few pages or on a single computer screen. The hazard register can capture and consider the operational environment and the various organizational capabilities (people, processes, technologies) that could degrade safety performance.

When used to track the implementation of mitigation actions and the resultant impact on risk ratings, the hazard identification register becomes a valuable communication tool by informing Top management of the progress or lack thereof and any additional resources required.

The hazard identification process considers all possible hazards that may exist within the scope of your operations and activities, including interfaces with other systems, both within and external to your organization.

The initial risk rating is the combination of the probability of a specific unwanted event happening and the consequences if they should occur. All hazards rated ‘Moderate’ or ‘Major’ must be prioritized for control and elimination.

Risk Assessment Form

The health and safety risk assessment form is a qualitative risk assessment (QRA) consisting of qualitative likelihood (probability) and severity (impact) scales Risk assessments are undertaken to ensure a detailed understanding of the effect of each hazard and risk that is rated as 'Moderate' and 'Major' within the Hazard Identification Register.

Risk assessments are undertaken with varying degrees of detail depending on the type of hazard and the information, data and resources available. It can be as simple as a discussion with your workers or involve specific risk analysis tools and techniques developed for specific risks or recommended by safety professionals.

SWOT analysis

A SWOT analysis can uncover areas of the business that are holding you back or that your competitors could exploit if you do not protect yourself. Many criteria will apply to more than one quadrant in the matrix. Identify the criteria appropriate to your own situation. The relevance of internal climate change considerations should be captured in this document.

PESTLE analysis

A workshop approach is often best when undertaking a PESTLE analysis. Organizations should include climate change as an external issue in their PESTLE analysis and make a statement on its relevance. This should be discussed during management review meetings and recorded in the review meeting minutes.
